Talbot Health Services has been proudly serving Ohio residents for over 10 years providing alcohol and drug prevention, education and intervention services. Our Mission is to save lives…One Life at a time, One Day at a time. We are now taking the next step in our service history by providing Addiction Treatment with a special emphasis on opiate addiction and mental health. Talbot Health Services MAT (medically assisted treatment) program combines prescription medications, counseling and behavioral therapies to address our clients’ substance abuse disorder (SUD) needs. The services we offer take a whole person approach to treating SUD. As a member of our Mansfield, Ohio, team you will be part of an Intensive Outpatient Treatment Program individualized to address a client’s needs.


The Counselor is a member of the Treatment Team. S/he will be responsible for coordinating the services needy by each client assigned to his/her caseload. S/he will provide individual and/or group counseling. The Clinical Supervisor will be the immediate Supervisor.


1. Abide by all clinic policies and procedures as documented in the Personnel manual and in the Policy and Procedure Manual.

2. Attends all staff meetings unless previously excused by administration or unless absent from work.

3. Abide by the professional ethics of the individual’s professional discipline and organization.

4. Will maintain full compliance with state/federal rules and guidelines regarding treatment.

5. Adhere to all rules of Federal Confidentiality.

6. Adhere to all requirements established by funding sources for maintaining case records.

7. Shall be responsible for the accuracy and full compliance of all chart documentation including, but not limited to:

· Biopsychosocial Assessments

· Diagnostic Summaries

· Treatment Plans

· Treatment Plan Reviews

· Progress Notes

· Grant Admission Forms

· Signed Release of Information Authorizations

· Follow-Up release forms

· Correspondence

· Discharge-Transfer Summaries

· After-care Plans

8. Must complete all billing information accurately, and within required time period, as applicable.

9. Will provide the following services under the direction and supervision of the Clinical Supervisor:

· Screening for appropriateness for treatment

· Orientation of client to program

· Assessment of client

· Treatment Planning

· Case Management

· Crisis Intervention

· Client Education

· Referrals

· Case Consultation

10. Maintain an “audit ready” status with prompt filing of case record documentation.

11. Continue to attend workshops, seminars, in-services, classes to remain current with the changes, revisions and additions within the profession.

12. Maintain communication regarding professional treatment of clients with all other departments and shall follow recommendations from other professionals when necessary and applicable.


To perform other duties as assigned by the Clinical Supervisor.


A Bachelor’s or Master’s degree in psychology, social work, or a related health field. Substance abuse experience/training preferred. Licensed to provide behavioral health services in Ohio. Also acceptable is and LICDC-CS as designated by the Ohio Chemical Dependency Credentialing Board.


1. Must possess demonstrated computer skills and knowledge in MS Office software applications including Word, Excel, Publisher and Power Point.

2. Must demonstrate the ability to effectively function as a team member.

3. Must demonstrate the ability to effectively manage multiple tasks concurrently.

4. Must possess exceptional communication and organizational skills.

5. Must be able to customarily and regularly exercise discretion and independent judgment in significant matters.

6. Must be able to relate with empathy and sensitivity to staff, clients and the community.

7. Has the ability to appropriately interact on the telephone and answer agency questions.


S/he should be licensed as one of the following: ISW, LSW, CSW, PCC, LPC, Chemical Dependency License or Licensed Psychologist.
