Job Description

This position will perform care coordination and outpatient behavioral health services for new individuals coming into services to ensure their success in recovery. The primary role will be ensuring individuals are linked from Open Access to their first outpatient appointment. Care Coordination team responsibilities include ensuring individuals receive timely access to quality care, working with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to care. The QMHP will ensure a seamless transition for individuals receiving services across a full spectrum of health and social services and will ensure that all gaps in care are reconciled within a trauma-informed, person-centered model of care. The QMHP will: maintain communication with care providers and coordinate transfer of medical records, prescriptions, and any follow-up appointments; interface with the outpatient behavioral health, substance use disorder and crisis teams to coordinate transitions between different levels and types of care; make referrals for services and supports to meet needs of the individual; coordinate or participate in multi-disciplinary team meetings to ensure all gaps in care are reconciled and other duties as assigned. This position reports to the Program Administrator for Open Access.

  • Bachelor’s degree in psychology, social work, nursing or related field from a four-year college or university that meets the Texas Administrative Code for requirements for a Qualified Mental Health Professional- Clinical Services (QMHP-CS).
  • May have a Master’s degree in counseling, social work, nursing or related fields.
  • Must have at least three years of clinical experience in the treatment of individuals with mental illness and/or substance abuse disorders.


  • Ensure individuals presenting to Open Access get timely access to quality care. This includes and is not limited to outpatient behavioral health, telehealth, primary care, substance abuse, crisis, and other services within LifePath and the community.
  • Participate in interdisciplinary treatment teams for individuals on caseload to identify gaps in services and opportunities for collaboration.

  • Understands and implements trauma informed care practices.


  • Experience in service coordination including community resources.
  • Ability to foster collaborative relationships with key interfacing teams, be a self-starter who works closely with all team members and takes initiative in completing tasks.
  • Ability to ensure effective communication with the multi-disciplinary team and across a variety of community resources.
  • Will work under general supervision with moderate latitude for the use of initiative and independent judgement as well as identifying when a licensed professional need to be consulted.
  • Knowledge of the social services system with the ability to connect individuals with appropriate services.
  • Must be organized, detail oriented, computer proficient, and able to write clearly and professionally.
  • Ability to respond with individuals and staff with patience, empathy, and understanding.


  • Must have valid Texas Driver License and a good driving record.
  • Must be able to pass a criminal background check.
  • Must be able to complete all LifePath Systems training.
  • Must be able to provide at least 3 professional or educational references.

Salary Range: $48,000 per year
Shift Hours: Monday - Friday 8am - 5pm
