Job Title: Program Supervisor

Desired start date: Immediate

Location: Hybrid with remote and on-site work: Applicant must reside in (or within commuting distance to) Central Valley, Central California, and/or Monterey Bay areas.

ModifyHealth changes lives by making food as medicine simple and sustainable. Our turnkey programs improve outcomes and costs for chronic conditions such as diabetes, heart disease, kidney disease, irritable bowel syndrome (IBS), and related GI issues where dietary management is recommended. We partner with providers, payors, employers, and patients to make adopting healthier diets and lifestyles a reality.

The Program Supervisor plays a pivotal role in successfully implementing and expanding CalAIM Community Supports services. The CalAIM Program Supervisor will be responsible for overseeing a team of Community Health Workers (CHW’s) as well as developing programs. The CalAIM Program Manager will ensure effective day-to-day operations and processes while supervising and developing staff. This position involves collaborating with ModifyHealth employees/leadership, local community-based organizations, health plans, etc. to provide integrated services and care management for patients. By monitoring and evaluating the delivery of community supports services, they can identify areas for improvement, implement quality improvement initiatives, and optimize the impact of these services on the target population. Additionally, the Program Supervisor will serve as a vital point of contact for individuals, providing support and guidance to promote their engagement and utilization of the CS services. Their expertise in project-based work and dedication to patient-centered care will contribute significantly to increasing the reach of the CalAIM CS services, ultimately improving health outcomes, and enhancing the well-being of individuals in need. This role must be linguistically appropriate to serve the diverse needs throughout California, by providing information and support in a manner that promotes understanding and inclusivity no matter the native language or education level of participants. The Program Supervisor ensures the delivery of high-quality care, enhances patient experiences, and is directly responsible for the overall success of the CalAIM program at ModifyHealth.

RESPONSIBILITIES AND DUTIES

  • Resource Allocation and Compliance: Coordinate resource allocation, oversee record-keeping, and ensure compliance with program directives, regulations, and guidelines.
  • Program Evaluation and Improvement: Evaluate program effectiveness, implement improvements, and modify operations as necessary to meet program directives and client needs.
  • Collaborative Activities: Develop, coordinate, and participate in collaborative activities, programs, and projects to maximize resources and client responsiveness.
  • Staff Supervision and Training: Provide supervision for all CHW’s, identify training needs, and ensure staff development programs address those needs.
  • Systems Management: Develop and manage systems to maximize program effectiveness, acting as a resource for staff and clients.
  • Community Collaboration: Collaborate with local agencies and organizations to achieve program goals and objectives within agreed-upon time frames and regulations.
  • Outreach Planning: Develop and manage an Outreach Plan, including scheduling and team coordination.
  • Travel and Participation: Travel for events, meetings, trainings, and conferences as required.
  • Cultural Sensitivity: Remain culturally sensitive and respectful in all interactions with clients and staff.
  • Confidentiality: Adhere to all client confidentiality requirements and standards.

QUALIFICATIONS AND SKILLS

  • Minimum of four years of supervisory experience in healthcare management, social services, project development, or related field
  • Proven proficiency in the area of program development and evaluation.
  • Experience with Microsoft Office products required (Excel, Word, Powerpoint, Outlook, Teams)
  • Fluency in English and Spanish (required)
  • Experience with billing systems and data analysis preferred
  • Knowledge of California Medicaid (Medi-Cal) and Managed Care programs preferred
  • Excellent organizational and leadership skills
  • Strong communication and interpersonal skills
  • Must maintain regular, punctual attendance and meet productivity standards
  • Valid California Driver’s License and auto insurance required

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
