Qualifications:
QMHP-CS providers must meet ALL of the following minimum credentialing requirements:
- Completed a standardized training curriculum
- Demonstrated competency in the work to be performed
- Obtained a bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
- Be a registered nurse (RN)
Responsiblites:
- Provide assistance in maintaining or improving functioning when necessary to help a client achieve a rehabilitation goal as defined in the treatment plan
- Assist clients in gaining access to needed medical, social/behavioral, educational, and other services and supports
- Provide community-based case management activities to clients or to the LAR on behalf of the client.
- If applicable assists the licensed staff in co-facilitating Group Psychosocial Skills Training to recipients, as assigned
- Completes CANS/ANSA as needed for all assigned clients.
- Participates in staff training assignments, as requested
- Assess, review, monitor a recipient’s status in relation to rehabilitation
- Submit accurate, legible and complete client progress notes
- Functions well in a crisis
- Must be under the supervision of a Licensed Practitioner
- Participates in the agency’s Performance Improvement initiative
- Attend/conduct all trainings, company and team meetings
- Understands, acknowledges and agrees to adhere to Medicaid.
- Acknowledges responsibility to participate in agency growth.
Job Type: Contract
Pay: $18.00 - $25.00 per hour
