Job Description:To provide Intensive In-Home, Mental Health Skill-building Services and/or Crisis Stabilization to individuals approved for services by their MCO/Medicaid. These services shall be rendered in accordance with FACES, Inc.’s policies and procedures and rules and regulations set forth for DBHDS and Virginia DMAS.
Desirable Minimum Qualifications:
- Bachelor degree in Human Services field
- Qualified Mental Health Professional Adult/Child/Trainee according to Virginia DMAS Regulations; must be registered with the Virginia Board of Counseling
- Ability to understand and interpret the policies, procedures, and general organizational structure of the agency
- Excellent verbal and written communication skills
- Available to work a flexible schedule that includes evenings and some weekends
- Valid driver's license and access to an automobile on a regular basis
Duties and responsibilities set forth for this individual include, but are not limited to:
A. Provides direct, IIH, CS, MHSS services to identified individuals and their families;
B. Assists in planning of services;
C. Provides parent education and support as needed to individuals who are receiving services and have children;
D. Assist and support with Adult Daily Living skills;
E. Models and teaches problem-solving techniques;
F. Provides a variety of interventions in individual’s home and community as appropriate and necessary. These interventions may be behavioral, social and recreational, as needed;
G. Provides linkage to community-based services and supports; provides advocacy and support for family with these services;
H. Coordinates scheduling of appointments, as needed;
I. Coordinates and provides transportation, as needed;
J. Available for 24-hour crisis intervention and family support;
K. Is responsible for the proper and timely documentation of all information required by the Department of Behavioral Health and Developmental Services, Department of Medical Assistance Services and FACES, Inc.;
L. Maintains regular and ongoing contact with referring agencies, as well as, other agencies involved in individual and family’s case;
M. Actively coordinates and plans discharge and aftercare plans with referring agency;
N. Attends interagency meetings, as necessary;
O. Attends and participates in team meetings;
P. Attends and participates in orientation, in-service training, and conferences as scheduled;
Q. Meets DMAS supervision requirements depending on the services being rendered;
R. Performs other duties as assigned;
S. Maintain and keep current all Human Resources documentation (ie: car insurance, car registration, TB test results, first aid/CPR, TOVA and any annual other trainings provided)
T. Attending annual trainings;
U. Complete documentation for individual files, such as: progress notes, individualized service plans, quarterly reports, discharge summaries and any other identified documentation.
