Job Description
Behavioral Pathfinder is seeking a qualified mental health professional to join our team. The Qualified Mental Health Professional (QMHP) will work under the supervision of LPHA to provide goal directed interventions to support and improve mental and behavioral health of eligible clients.
QMHP-CS providers are eligible to deliver MHTCM and MH rehabilitative services and must meet the following minimum credentialing requirements:
- Completed a standardized training curriculum
- Demonstrated competency in the work to be performed
- Obtained one of the following a bachelor’s degree from an accredited college or university with a minimum number of hours that are equivalent to a major in
- psychology
- social work
- medicine
- nursing
- rehabilitation
- counseling
- sociology
- human growth and development,
- physician assistant
- gerontology
- special education
- educational psychology
- early childhood education
- early childhood intervention
- Registered Nurse or RN
- Must have and be able to conduct CANS/ANSA assessments
- Must be able to work with clients/patients and LAR in home, school, office or community setting.
- Must follow and adhere to all HIPPA/FERPA guidelines.
Job Description:
- Provides structured face-to-face scheduled sessions to client based on the treatment plans
- Provide Medication training and support services that consist of education and guidance about medications and their possible side effects.
- Provide Skills training and development services to an eligible client in addressing problems that interfere with the individual's functioning and learning environment.
- Provision Crisis intervention services and interventions in response to a crisis to reduce or manage symptoms of a SED and to prevent admission of an individual to a more restrictive
environment.
- Provide community-based case management activities to clients or to the LAR on behalf of the client, where applicable.
- Identify, establish, and continually collaborate with community agencies, providers, and natural supports and advise team to make referrals as needed
- Assist clients in gaining access to needed medical, social/behavioral, educational, and additional required services and supports
- Perform other related duties and participate in organization periodical meetings.
- Document treatment procedures and interventions in the organization EHR system.
Job Type: Contract
Pay: $23.00 - $25.00 per hour
