Case Management Care Coordinator/ EPCGC

San Elizario/Clint

Job Summary

The Qualified Mental Health Professional will provide Targeted Case Management services to qualifying Center clients. Care Coordinator will be trained in and deliver family-oriented, individual and/or group psychosocial rehabilitative services, skills training, medication training, and/or case management services according to best practice standards set forth in the Texas Recovery & Resilience Guidelines.


Essential Duties and Responsibilities

  • Complete training and certification in all evidence-based methodologies plus all versions of CANS and ANSAs, not limited to but including Wraparound, when applicable.
  • Maintain all certifications valid, renewing training and re-certification annually or as needed.
  • Become certified as QMHP-CS and participate fully in credentialing process with EPCGC.
  • Maintain caseload of TCM clients in care, completing also associated administrative and documentation tasks.
  • Conduct CANS/ANSA assessments with clients, ensuring timely completion of all assessment and intake paperwork.
  • Ensure all daily documentation is completed and entered all required databases in a timely and efficient manner.
  • Provide crisis intervention and safety planning for youth and their families as needed.
  • Develop and document treatment plans, including goals and objectives, on a timely basis or no later than every 90 days.
  • Based on client need, provide skills training, case management, psychosocial rehabilitation and/or medication management services, in alignment with the client's treatment plan and authorized level of care units.
  • Always maintain client confidentiality and be familiar with all HIPPA laws and regulations.
  • Participate in interdisciplinary case staffing meetings, trainings, and supervision as appropriate.
  • Complete all reporting tasks related to service and program evaluation, including participation in annual PQI processes.
  • Maintains compliance with all policies and procedures of the EPCGC, including compliance with COA standards and all other regulatory requirements.


Additional Eligibility Qualifications

  • Excellent verbal and written communication skills, required.
  • Must possess and maintain a valid state Driver's License with an acceptable driving record, required.
  • Must be able to pass criminal background and drug screen, required.
  • Excellent organizational skills and attention to detail, preferred.
  • Proficient with Microsoft Office Suite, preferred.
  • Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
  • Staffing requirements, including criteria that staff have diverse disciplinary backgrounds, and are culturally and linguistically trained to serve the needs of the clinic's patient population, preferred.

Work Environment/Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is regularly required to talk or listen. Prolonged periods of sitting at a desk, working on a computer, and using standard office equipment. This position requires the ability to travel throughout the community and occasionally must be able to lift office products and supplies, up to 20 pounds at times.


Work Schedule

The work schedule is variable from 8:00 a.m. to 8:00 p.m. Monday through Saturday.


Travel

50% travel is expected for this position.


Required Education and Experience

Bachelor’s degree from an accredited college or university and a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
