Job Title: Lead Care Manager - I (LCM-I)
Status: Part or Full-time (20 to 40 hours weekly)

SUMMARY
Under direct supervision of the Leadership Team, the LCM-I is responsible for evaluating the perinatal client as a total person and identifying strengths and challenges impacting participants’ overall health and well-being. LCM-I is primarily responsible for delivering Enhanced Care Management (ECM) services to perinatal clients but may also be assigned CalAIM Community Supports caseloads and assist with ECM program duties. LCM-I will conduct outreach services, member enrollments, and other duties as assigned.
ESSENTIAL DUTIES
1. Assist with conducting in-depth assessments/reassessments covering psychosocial, rehabilitation, and environmental concerns, with Care Manager oversight depending on program requirements.
2. Participate in developing the care plan, implementing the services, and conducting follow-up and monitoring of client’s needs.
3. Refer clients to community resources, and/or make recommendations to improve the client’s functional level, access to services, and address safety, and advocate as needed.
- Assist clients to identify and develop support systems addressing social determinants of health.
- Consult with the Clinical Consultant, as appropriate.
6. Perform outreach to potential clients, including screening, enrollments, or other program requirements.
7. Conduct community outreach and education activities.
- Function as part of an interdisciplinary team.
- Participate in trainings, conferences, or health fairs as appropriate to job duties or required by funding source(s).
10. Deliver CalAIM Community Support services to enrollees and prospects, as required.
11. Document Member status, encounters, contacts, or other metrics in various CalAIM-related websites, portals, folders, and platforms.
12. Ensure timely documentation of all encounters, community and provider outreach, and member follow-ups.
13. Schedule and ensure timely Member visits, appointments, and check-ins according to Member acuity, ECM program requirements, and Member preferences.
14. Maintain travel logs in Outlook Calendar, or elsewhere as directed.
15. Willingness to travel within the entire CalAIM service area as required.

· Must clear Department of Justice background check.
· Compensation based on experience, education, and program allocations.
· Experience working in or knowledge of Sacramento and surrounding counties (El Dorado, Placer, Sutter)
· Ability to serve members in person and remotely.
EDUCATION AND/OR EXPERIENCE
· This position requires a high school diploma or GED, plus a valid California Driver’s License, reliable transportation, and current vehicle insurance. Should have at least one year’s experience working in the community and must reside in and share experience, language and/or culture of the community to be served. Community Health Worker certification preferred but not required.
- Strong communication and organizational skills.
- Must be able to maintain ECM and/or CS Caseloads while ensuring that ongoing engagement and documentation is compliant with all program requirements and Member need.
- Ability to work as a functional and collaborative member of an interdisciplinary team.
· Requires a positive attitude, desire, professionalism, and willingness to work with low-income, medically compromised individuals, and an ability to work with participants, peers, community members, and collaborative partners.
- Proficient technical knowledge with current/prior versions of Microsoft and Apple operating systems, Office applications, Outlook/Exchange, Microsoft SharePoint.
· Demonstrated understanding of and genuine commitment to the diverse populations we serve, as evidenced through relevant lived or professional experience.
LANGUAGE SKILLS
Fluent in English with good communication and writing skills, bilingual preferred.
MATHEMATICAL SKILLS
Ability to perform basic arithmetic calculations required for tracking purchases and office tasks.
REASONING ABILITY
Ability to use good judgment to conduct professional and collaborative activities and solve practical problems within established limits and guidelines. Ability to recognize instances in which consultation with Program Supervisors, Directors, or Senior Director should be sought.
Interviews will take place virtually on April 23rd and 24th.

Pay: $20.00 - $23.00 per hour
