Position: Home Address Utilization Associate for Case Coordination
Location: South Dakota (Remote/Home-based with Travel)

Travel: Some travel required within South Dakota for case coordination (mileage reimbursed)

Job Description:

Use your home address as a business address for receiving correspondence related to case coordination.
Assist in coordinating care and services for individuals with developmental disabilities.
Travel to client locations or designated meeting spots within South Dakota for case meetings and coordination.
Maintain detailed records and documentation related to case management activities.

Requirements:

South Dakota resident with a valid home address.
Reliable transportation and willingness to travel for case coordination within South Dakota.
Strong organizational skills and attention to detail.
Compassionate and patient demeanor when working with individuals with disabilities.
